Estate Planning Client Intake Form
If you are looking to discuss getting a Last Will and Testament done, please fill out this form and bring it with you to your initial meeting.

Living Wills
This is a Living Will, also known as an Advance Medical Directive. This is a form put out by the State. It's a very misunderstood form. So we provide it here so you can read it prior to your initial visit.

Probate Client Intake Form
If you need a succession done for a loved one, or have been appointed as Executor or Administrator, please fill out the form with your information as well as the decedent's.
